Request for replacement equine passport

This page is to order a duplicate equine passport in case your horse's got lost.

ICS can only provide for a duplicate in case:
- the original equine passport was produced by ICS Nederland (Irish Cob Society)
- the original equine passport was produced by SHS (Shire Horse Society)

If ICS Nederland, or another Dutch studbook, issued the original passport is to be checked HERE using the transponder (microchip) number.
A duplicate passport costs 115 Euro, this includes postage costs.
Keep your credit card or Paypal reference ready as you will be forwarded to a payment page after sending in your data.

We cannot make a replacement passport:
- If the (foreign) passport issuer can be traced, they are supposed to make the passport.
   This is the case when the horse's life number does not start with 528023.
   You can find where to look, using the life- or chipnumber HERE.
- If the horse has an Irish life number that starts with 372414
   or a chip number starting with 3721414, then apply for a duplicate at Horse Sport Ireland,

- If the horse has an Irish life number beginning with 372003
   or a chip number beginning with 3721403 then apply for a duplicate passport to
   Leisure Horse Ireland (LHI, formerly IPSA), using THIS form.

Owner data
